1 Actual layout as well as the survey contents of the questionnaires attached here are slightly different from those which were actually used. There were some survey items which are pertinent to Indonesia only. There were also some other survey items which were surveyed but did not give much useful meanings. These were omitted in the attached questionnaires.

2 This questionnaire is not included here, as the basic layout is exactly the same as that of H 1.

3 This is an excerpt of the questionnaire used for the interview of agricultural households. The questionnaire for the agriculture households was designed so as to facilitate data processing by electronic computer, therefore, the actual layout of this questionnaire is not the same as the one given here.
